1- Internship application form (3 copies)
2- Copy of ID card
3- Student certificate
4- Printout of the insurance document provided by your school
5- Original or photocopy of the Occupational Health and Safety Training Certificate (training must be 16 hours) (the validity period of the training is accepted as a maximum of 1 year),
6- Blood test results including health screenings conducted within the last 6 months (Glucose, AST, ALT, GGT, HBsAg, Anti-HBs, Anti-HCV, Anti-HIV, Complete blood count, Blood type, Measles IgG, Varicella IgG, Anti-rubella, Mumps IgG)
7- Chest X-ray (with report)
8- An account number must be obtained from Ziraat Bank.
9- At the end of the internship; the student must submit a copy of the Attendance Sheet and Evaluation Form to the Education Unit.
10- The student must submit the required documents to the Hospital Education Unit at least 15 (fifteen) days before the start date of the internship/clinical practice training.
11- For Radiology and Medical Imaging students (Glucose, AST, ALT, GGT, Thyroid USG, Peripheral smear, Thyroid function tests, HBsAg, Anti-HBs, Anti-HCV, Anti-HIV, Complete blood count, Blood type, Measles IgG, Varicella IgG, Anti-rubella, Mumps IgG) blood test results
